As of 2025, tuberculosis (TB) remains the world’s deadliest infectious disease, surpassing HIV/AIDS. According to the latest WHO data, the global TB incidence was around 134 cases per 100,000 population in 2023—still alarmingly high despite long-term declines.

1. Global Epidemiology and Incidence

WHO data indicate that TB incidence declined from 180 per 100,000 in 2000 to about 134 per 100,000 in 2023. Despite this progress, TB remains a major health threat in Asia, Africa, and parts of Eastern Europe. Kyrgyzstan, for example, reported an incidence of 112 cases per 100,000 population in 2023, ranking among the world’s highest TB burden countries.

2. Prevention and Vaccination Strategies

1) Current BCG Vaccine

The BCG vaccine remains widely used, especially for infants, and is effective in preventing severe forms of TB such as TB meningitis and miliary TB. However, its efficacy against pulmonary TB in adolescents and adults varies by region and age group, limiting its long-term impact.

2) Next-Generation Vaccine Development

The Spanish-developed MTBVAC vaccine has emerged as a promising candidate, offering potentially 50% greater effectiveness than BCG. Currently in Phase 3 clinical trials, it targets commercialization by 2029 and aims to protect both infants and adults.

3) M72/AS01E Candidate

The M72/AS01E vaccine developed by GSK, supported by the Gates Foundation and Wellcome, entered Phase 3 trials with results expected by 2028. If successful, this could be the first adult TB vaccine in over a century.

3. Global Policy Strategies for TB Elimination

WHO has set ambitious goals to reduce TB deaths by 95% and incidence by 90% by 2035 compared to 2015. However, as of 2025, progress has been far too slow. World TB Day 2025 was observed under the theme “Yes! We Can End TB,” emphasizing renewed political commitment and community engagement worldwide.

4. Multidrug-Resistant and Extensively Drug-Resistant TB

Multidrug-resistant TB (MDR-TB) and extensively drug-resistant TB (XDR-TB) are major challenges. Treatment remains lengthy, expensive, and often toxic, leading to poor adherence and high public health risks. MDR-TB rates are increasing in Central Asia and Africa, making new treatment regimens and monitoring systems critical.

5. Financing, Policy Gaps, and Opportunities

  • Underfunding remains a major barrier to scaling up diagnosis and treatment. WHO estimates a multi-billion dollar annual funding gap for TB programs.
  • Partnerships with high-burden countries such as those in Central Asia are strengthening efforts to eliminate TB by 2030, with support from global health organizations.
  • Public-private partnerships and new diagnostic technologies offer opportunities for more effective TB control.

Tuberculosis continues to be a global health crisis in 2025. Current BCG vaccination has limitations, and the rise of drug-resistant TB adds urgency to the fight. However, promising candidates like MTBVAC and M72/AS01E, combined with WHO’s elimination strategy and stronger global investment, offer hope. Achieving TB elimination requires a coordinated approach that integrates vaccines, treatment, diagnostics, and policy reforms on a worldwide scale.
